If a person’s condition requires they be transferred to St. Louis for additional services, we will coordinate that care with their primary physician and ensure they are sent back to the community for any follow-up care.

Continuity of Care
Communication with patients’ primary care providers and continuity of care for patients is of utmost importance. The Heart Center pairs its doctors and patients so together they walk the patient’s journey to recovery.

METS

The team of experts in Monitored Exercise Testing Services (METS) is dedicated to helping patients who have (or are at risk of developing) heart disease, lung disease or diabetes.

The METS team is comprised of doctors, nurses, exercise physiologists, respiratory therapists, dietitians and certified diabetes educators. Team members help people identify the right wellness programs for them and then get them back on their feet to create a lasting, healthy lifestyle.

Cardiac Rehabilitation
Cardiac rehabilitation is an exercise and education program for people who have heart disease or who have had a heart attack or a heart procedure such as angioplasty, stenting, bypass surgery, valve replacement surgery or heart transplant. Rehabilitation is a strategy for creating a heart-healthy lifestyle to prevent problems from recurring.

Pulmonary Rehabilitation
Pulmonary rehabilitation is an exercise and education program for people who have chronic obstructive pulmonary disease (COPD), emphysema, asthma or other serious lung conditions. Although there are no cures for these diseases, there are effective therapeutic techniques that can help improve health, decrease hospitalizations and improve quality of life.

During cardiac or pulmonary rehabilitation,

  • a team of experts evaluates participants’ conditions and develops individualized exercise programs.
  • participants’ heart rates and rhythms (or lungs are continually monitored so individual programs can be fine-tuned along the way.
  • participants report that receiving education while they exercise makes classes more enjoyable and encourages support, interaction with other class participants and the sharing of ideas.

Living with Diabetes
This exercise and education program is designed for people with diabetes – from those who have just been diagnosed to those who have been living with diabetes but who want to be healthier and feel better. Participants work one-on-one with a registered dietitian and a registered nurse, both certified diabetes educators, to evaluate their health statuses, including completing a full nutrition assessment, to find a path forward. For more information, email Paula Enstrom, or call 217-238-4808.

Join In!

The Heart Center’s programs are fun, engaging and motivational, and they provide participants with the tools they need to take control of their health! Most classes meet two or three days per week, and many are covered by insurance. Some classes have a small fee. The staff is happy to help those who are interested determine which class is the right fit and what the costs might be, if any.

Please call METS for more information, or to join a class at 217-258-2334.
