Program/Service Notes:
The cardiac rehab program at Horizon Health is specifically designed to meet the individual needs of each patient recovering from a variety of cardiac conditions occurring in the last 12 months. Conditions include:
Heart failure
Heart attack
Coronary artery bypass surgery
Stable angina pectoris
Heart valve repair/replacement
Heart or heart-lung transplant
Angioplasty
Arrhythmia
