Program/Service Notes:
Care managers conduct a free comprehensive assessment in the home of the older adult and provide linkage between client and community based services. Eligibility is determined for Illinois Department on Aging Community Care Program designed to prevent or delay nursing facility placement with services such as Home Care Aid, Adult Day Service and Emergency Home Response Services. Care managers also conduct the universal pre-screening on all entering a long term care facility.
