Program/Service Notes:

This Social Adjustment and Rehabilitation
Program provides individual counseling and treatment planning to those age 60 and
older and living independent of a skilled long-term facility. The goal of the
program is to improve the quality of life and maximize independence and dignity for
older Americans. Services shall also include client-centered consultation, a
comprehensive mental health needs assessment, as well as advocacy linkage and
supportive case management. Services are usually provided in the home of the older
adult at no cost.
