Eligibility:	Eligibility is based on a multidisciplinary approach and the evaluation and assessment processes help teams determine eligibility, identify the unique strengths and needs of a child and family, and identify the appropriate services to help meet those needs.
Intake Process:	A service coordinator conducts an intake with a family after a referral is made. Call 888-459-5437 Ext. 101 to make a referral

Child and Family Connections is a statewide system established to ensure that all children under the age of three, who are identified as having a possible developmental delay or have a diagnosed disability that may result in a developmental delay, are referred to determine eligibility for early intervention services.
